  • Organized at Valcartier in September 1914 under the command of Lieutenant-Colonel A. T. Shillington
  • Most of the recruits were from Nos. 12, 14, 15 and 19 Field Ambulance of the Active Militia
  • Left Quebec 30 September 1914 aboard SCOTIAN
  • Arrived in England 14 October 1914, Strength: 8 officers, 86 other ranks
  • Arrived in France 8 November 1914, At Le Touquet, November 1915 – September 1915, At Outreau, October 1915 – April 1919
  • Ceased operations 1 April 1919, Disbanded by General Order 211 of 15 November 1920
